California Subacute Rehabilitation Services
Above and Beyond Congregate Living is a licensed 24-hour Congregate Living Health Facility (CLHF) in California, providing individualized subacute care in a safe and welcoming residential environment. We support adults with complex medical needs, including spinal cord injuries (SCI), traumatic brain injuries (TBI), pulmonary and neuromuscular conditions, congenital diagnoses, and other medically fragile situations.
Our dedicated care team consists of licensed nurses, social workers, activity director, dietician, physical therapist, occupational therapist, speech language pathologists, and more, guided by our medical director to create a care plan that ensures each resident receives personalized support and daily care to meet their needs. Beyond medical care, we prioritize comfort, dignity, and emotional well-being. We take pride in helping residents feel at home while giving families confidence and peace of mind.
